Orientation begins on Monday, June 8 in Hoffman Estates, IL and is NOT optional. Do you have any school or work conflicts that would prevent you from arriving on time? If so, please explain.*

By submitting this application, I acknowledge and agree to each item.
- I will complete all pre-field training assignments and medical exams in a timely manner.
- I am available to attend Summer Mission Team orientation from February 13-16, at Wonderland Camp in Wisconsin.
- I am available for assignment from June 8-July 26, 2026.
- I agree to raise $800 for program costs. A non-refundable $150 deposit will be due on February 14. The remainder will be due at the end of May.
- I am willing to serve as a member of a Summer Mission Team as assigned.
- I am willing to wear a Salvation Army uniform (if a soldier) or a white Salvation Army polo/black pants as requested while on assignment.
- I understand that drinking alcohol, vaping, smoking, or using any other drugs is not permitted during SMT orientation, on-field ministry, or SMT debriefing.
- I understand that if chosen to minister on an SMT I may be eligible to receive a $3,000 educational scholarship from The Salvation Army upon satisfactorily completing the ministry assignment.
- Team members are responsible for making their own travel arrangements to orientation on June 8 and departure on July 26.
